For students in grades 7-12:
  • Parents/Guardians living within the Fillmore Central School District must fill out a School Entrance Form.
  • For your convenience, you can print out an entrance form by clicking on the link below. 
 
  • A copy of your child's last report card, transcript (8-12th grade), birth certificate and immunizations would be helpful to have when you come to register.  
 

If you are a parent/guardian not living within the Fillmore Central School District and would like to enroll your child, you must fill out an out of district form.
